The following death notice is from The Cullman Tribune of Cullman, AL, 27 May 1915: DR. J. W. CULPEPPER MET WITH FATAL ACCIDENT Tuesday When His Automobile Turned Over Into a Ditch on Third Street Resulting in His Death Thursday Morning. Dr. James W. Culpepper met with a deplorable accident on Tuesday about 1 o'clock, that resulted in the death of the well known and popular physician at the Davis infirmary in Birmingham Thursday morning at 9:45 o'clock. The accident which claimed this great man occurred when his automobile left the street and plunged into a ditch throwing the Doctor out of the car turning up on its side and falling on him from which he sustained the fatal injuries. Dr. Culpepper had left the People's Drug Store and was en route home for the noon hour meal. While driving rapidly in an old Hudson car and after turning over the hill from J. R. Griffin's store one of the cushions was about to fall out of the car and he reached down to pull in the cushion in place when he lost control of the car and almost in an instant the car was plunged into the ditch on the north side of Third street in front of J. F. Weeks store. Mr. Weeks who witnessed the sad accident rushed to the rescue of Dr. Culpepper and as best he could assisted the Doctor up out of the ditch on to the bank where Dr. Culpepper made a thorough examination of his injuries the second time and then remarked to Mr. Weeks "I am a dead man, send for my wife at once." Kind and sympathetic friends gathered around to do whatever was within their power and Drs. Hays, Martin, McAdore and Thurlow at once rushed to the side of the Doctor to give whatever assistance that could within the skill of medical science. The injured man was carried to the Cullman Sanitarium and after an examination by the attending physicians it was decided to carry him to Birmingham where a more thorough examination could be made with the X Ray and if possible something done that would save his life. Accompanied by Mrs. Culpepper, Drs. Hays, McAdore, Thurlow and Otis Kinney he was carried to Birmingham on the evening accommodation and on Wednesday morning the X Ray examination made, but it was found that his condition and injuries were such that an operation would be of no avail as the left hip was crushed all to pieces and he [was] badly injured internally. The Doctor lingered between life and death until Thursday morning when his spirit of life was wafted to the great physician in the great beyond. The remains accompanied by relatives and friends were brought to Cullman Thursday afternoon and Friday afternoon at 3 o'clock the funeral services will be conducted at the Baptist church after which the body will be laid to rest in the city cemetery. No death has ever occurred in our county that was more universal[ly] deplored for Dr. Culpepper was a man held in the highest by all who knew him. His wonderful skill and success as a physician was such as to demand all of his timely, rarely ever stopping day or night, it making no difference from whom the call came for assistance, whether great or small, he was ever ready to answer the summon, his life being in his work of doing whatever was within his power for elevating and helping suffering humanity. This great physician and kind hearted gentleman will be sadly missed by not only his dear family but by hundreds of people throughout the county. Dr. Culpepper was near[ly] 39 years old, and is survived by a widow and two sons, Wallace and Voight, father and mother Mr. and Mrs. Thomas J. Culpepper of Crane, four sisters, Mrs. Lena May Price, Mrs. Sally Land and Mrs. Lillie Tucker of Crane Hill; and Mrs. Hnlen Birch [read H. L. (Ellen) Burch] of Florida; two brothers, J. P. Culpepper of Crane Hill, and Dr. R. A. Culpepper of Chicago.
      The following funeral announcement is from an unknown source: D I E D In Birmingham, Ala., at 9:45 o'clock a. m. Thursday morning, May 27th, 1915, DR. JAMES W. CULPEPPER. Funeral from the Baptist Church, Friday atternoon, May 28th, 1915, at 3 o'clock p. m., conducted by Rev. W. P. Wilks. Burial at Protestant Cemetery. Friends and acquaintances of the family are requested to attend. Pall Bearers: Dr. L. Hays, Dr. J. C. Martin, Dr. E. D. McAdory, Dr. A. A. Thurlow, Dr. W. J. Cottingham, Dr. O. W. Kinney.
