STATE OF GEORGIA, County of Webster
In person came before me, the undersigned Ordinary Mrs. Nancy R. Culpepper, who being sworn according to law, says under oath that she is the widow of Joseph F. Culpepper, who was a soldier in the service of the Confederate States, and served as a member of Company B, of the 11th Regiment of Georgia Volunteers, that he enlisted in said service on or about the 3rd day July 1864, and was in the army up to September 1864. That while in the Army, he was on the 14th day of September 1864 taken sick with measles and died at Ocmulgee Hospital at Macon, Georgia.

Affidavit for Three Witnesses
STATE OF GEORGIA, County of Webster
In person came before me, the undersigned Ordinary in and for said County, witnesses J. B. Goss, J. R. Stapleton, and A.J. Smith (each known to said Attesting Officer as truthful, reliable and reputable citizens), who severally say oath, that, from their own personal knowledge, Mrs. Nancy R. Culpepper, of the County of Webster, State of Georgia, is the widow of Joseph Franklin Culpepper, who was a soldier in Company B of the 11th Regiment of Georgia Volunteers. That said soldier enlisted in the service of the Confederate States (or the GeorgiaState Troops) on or about the 14th day of September 1864. That while in said service, or by reason of said service in the Army, he lost his life as follows: Was taken sick with measles and died from the effects of measles.
James R. Stapleton comes before me and on oath says that he was in the same company with the said Joseph F. Culpepper and that while in service he was reported sick and soon afterwards was reported dead and he has never seen nor heard of the said Joseph F. Culpepper since and that he is satisfied that the said Joseph F. Culpepper died at the time he was reported to have died and that he had lived near by the family of said Culpeppers ever since the surrender.
J. B. Goss on oath says that he was a comrade of Joseph F. Culpepper in the Confederate service and that at the time of his death he was in the service and was in the City Hall Hospital at Macon, Georgia and died on or about the 14th day of September 1864 and that he the said Goss said the said Culpepper died and he carried his body out of the ward after his death.
P. L. Christian further swears that he knows the said Joseph F. Culpepper was in the City Hospital very sick and that he was reported dead and if he should not have died he has ever since been unknown and that his is just as certain that the said Joseph F. Culpepper died while in the service of the Confederacy.
